At New Heights Service Solutions, we provide support based on the Individual Plan of Service (IPOS) or Person-Centered Plan (PCP) developed and managed by the individual’s assigned support coordinator or case manager through their designated support coordination agency.
We collaborate with various agencies, including those within the Oakland Community Health Network (OCHN), to ensure individuals receive high-quality, person-centered care. These collaborations enable us to deliver tailored support that aligns with each person's unique needs, goals, and preferences, empowering them to live as independently as possible while receiving the assistance they require.
Our role is to implement and follow these plans with a compassionate, individualized approach, assisting with daily living, community engagement, and skill development. Additionally, if an individual chooses to work with our staff on residential or community-based goals, we actively participate in plan meetings and goal-setting discussions. This collaboration allows us to provide direct input while maintaining coordination with the assigned support coordinator or case manager to ensure seamless support.
